What went well?
• Most of the event of 3rd quarter were accomplished
• IEC material properly used at Upazila level
• During the visit, it was observed that leprosy information sign board already been installed at UHC and Birganj mor (public place) at Birganj Upazila
• Observed family planning health workers orientation at FWC office, Birganj, Dinajpur

Major finding:
• Invitation letter, Schedule, Minutes/Report did not maintain as per methodology
• Integration project staffs of DLC received honorarium as facilitators but they are not entitle as per methodology.
• Without any monitoring visit plan, schedule, bill voucher and field report DLC staffs received supervision and monitoring fees.

Challenges (implementer):
• Ensuring Govt. resource person in events
• Govt. staff did not give time to attend orientation

Feedback/recommendation:
• Each event will be documented as per methodology
• Since DLC project staffs are not entitle to get honorarium, they can receive only travel bill and food cost.
• Before supervision and monitoring visit must be prepared travel plan, movement register and visit report.
• Maintain separate actual bill voucher during the monitoring visit time.
• Recipient must be sign on the master roll which money they actually receive.
• Maintain register khata and file to document various events
• Event photo for all the events must be kept (soft copy)
