Please check my letter to my dentist

Dear Doctor

I have received a copy of your records. On page 1 of the records, I see that the dates of the three entries are the same, that is dated January 15, 2011: That is incorrect. I have made notes of my visits. The correct dates should be February 15, 2011, March 15, 2011 and May 15, 2011.
