Letter Writing

Please check my grammer

Hello everyone,

I resigned my job suddenly, without giving the requisite notice period, two months back due to an accident. I informed my boss about my accident. Also, I said that I would like to resign from my job. My boss accepted my resignation and paid me what I was owed. I worked in a job which entailed one week of morning shifts and one week of afternoon shifts. Now I have recovered somewhat, I would like to go back to work but I can do only fixed morning shifts because when I have an afternoon shift, I have to travel at night. I don't want to travel at night because I had my accident at night. Also, I come a long distance in heavy traffic. If my boss accepts my request I would like to work only the morning shift.

I need to write a letter about this to my boss.
Please help me

Dear Sir,

First, I would like to say thanks for my July salary. I am writing this letter to request to rejoin the company. As you know, I resigned from the company two months back due to the leg injury I sustained in a road accident. I have now recovered.

I would like, if possible, to come back to work but I would be able to work only the morning shift. This is because to work the afternoon shift, I would have to travel at night. I don't want to travel at night as that is when I had my accident. Also, I have to travel a long distance in heavy traffic.

If it is possible to arrange only morning shifts for me then I would be able to return to work.

Thank you for considering my request. ( Hope you consider my request) which one is correct

Yours faithfully,

xxxxxx

Is this formal?
Please make any necessary changes.

Thanks in advance.
