Please check grammar of my CPD
PharmOutcomes is a web-based system which helps community pharmacies provide services more effectively and makes it easier for commissioners to audit and manage these services but we use this system mainly for our patients who use a compliance aid:
to notify their pharmacy about their admission and
to send a discharge notification when they get discharged from the hospital.
proper communication between between the hospital and the patient's pharmacy is critical to providence safe and effective care and to avoid any preventable errors such as starting or stopping therapy which is against the latest discharge letter. Generally the Pharmacy support workers do this task but on that weekend I was alone and had to send an urgent discharge referral to the patient's pharmacy so that they can prepare a compliance aid quickly as per the discharge notification letter. I asked one of my colleagues who knew how to use the system so managed to send the discharge notification quickly as the patient was already registered. I know now who to contact in case of any problems to use the system.
