Is it okay to say, "He is playing the piano very hard," to mean that he is playing the piano earnestly.
Does "He is playing the piano very hard" mean he is playing it with a strong touch?

I'd take it the second way.

But you could say he is studying hard.

"Earnestly" could mean with great intensity - not necessarily with great force.

"He is playing the piano very hard" is not an expression I've ever heard.

Perhaps, "He is pounding on the piano."
