Place of Adverb in an Interrogatory Sentence.

Which sentence of the two is correct:
"What happened actually?" or "What actually happened?"?
If both are, is there a difference in meaning?

Anonymous What happened actually? Not usual, but understandable as meaning the one below. I don't recommend it except possibly with a pause for rhetorical effect.
