Picture someone as

How would you phrase the last sentence ?
He has Alzheimer's. Every time he sees his wife, he tells her she's not his wife because his wife is pretty and she isn't. I guess he pictures imagines sees his wife as a 20 year old bombshell she once was.

Just considering the last sentence: "... " You could also use 'sees' just as easily, but since it's a mental image 'pictures' is better as 'see's implies he can physically see her that way. For imagines, it wouldn't be my preference as it implies something he actively tries to imagine in his head, rather than him seeing a memory.
