"Yes. Who's this?"

"This is April's mom."

"Oh... Right."

Hi.

1) I know that you'd typically introduce yourself when you pick up the phone, but if someone doesn't do that, is this natural?

2) The intention is that Frank has no idea who April or her mom is, does "Oh... Right." imply that?

Phone manners depend on the culture and in the days of VOIP, this has changed. In the US, we get many robocalls, so most people do not announce their name when (if) they answer.
