Personal Sponsorship

Hi, I'm looking for a format to send to a company (indoor ski center) to ask for a sponsorship for my sun who trains and competes at there venues on a regular basis.

The staff at the company have put his name forward and have told me to write to them in a form maner asking for a full sposorship deal for my son.

Any help would be welcome.
