Personal Leave

I currently need help writing a letter to my employer asking for personal absence. My employer is aware of my situation however they need a letter to place on my file.

I am currently requesting for 6 months off work to be by son's side. However within those six months i will be available to work from home if needed. (On Call)

this is what i have so far.

Dear N/A:
The purpose of this letter is to request Leave of absence from my position with company starting as of May 13th.The reason for this request is due to my situation with my son, and having to continuously take him to therapies.

I will make sure that you and any other employees who may be impacted by my leave of absence are aware
