Can you use:

We are patrons at X hospital. Or does it have to be:

We are patients at X hospital.

And does it have to be with the at preposition.

What about if it's like a cosmetic surgery clinic? or a laser eye surgery clinic where there's a lot of clients and customers..etc..

In a context dealing with the medical aspects, 'patients' is better.

In a context dealing with spending money, and even perhaps with repeated visits, 'patrons' seems OK.
