HISTORY:
patient is a 54 yrs old female with a BP.

PAST SURGICAL HISTORY:
She underwent fistula operation in 02/02/1999. Underwent sugery for U Hernia.

CURRENT MEDICATIONS:
Aten 50 mg daily at 9am.
lybotryp once a day at night time.
cremaffin white.
Neurobion IM/ once in a week for 6 weeks.
FAMILY HISTORY:
niddm, piles.
PHYSICAL EXAMINATION:
Respiratory exam is clear to auscultation. Heart normal in size , rate and rhythm. Normal S1 and S2.
No murmur. Extremities reveal edema due to BP drug.

with a BP? probably okay but I am not a doctor and do not know what a BP is- usually makes me think of blood pressure 54-yr old a fistula operation Neurobion/IM/once a week [six-week course] seems basically ok- not sure what you are worried about as it is more like a list than a text....

Oedema - British/Indian way of writing.
