No,the answer is number 4. The clue is 'since last Tuesday' as that infers any time between last tuesday to the present, so it would have to be written 'who have come into contact...'

If you changed it to 'anybody' instead of a specific group of people, you would write it as 'anybody who has come into contact...' not 'anybody who had come into contact...' - that should make it clearer.

Sentence #4 is correct from the past tense verb usage. However, actually none of the sentences are correct because: "...last Tuesday, will undergo medical tests tomorrow. Or it could be phrased "will undergo medical testing tomorrow.
