Past simple/past continuous/past perfect

How long had they lived together before they got engaged?

How long were they living together before they got engaged?

Which of these two is correct? I believe that the first one but the second one is also very common.

How long did you live/were living in London?

Here, I'm not sure which one is ok. I think it is possible that both are ok in spoken language but in written I'd use Past Simple, since I'm no longer living there.
