But what had really happened to him after that stroke?
Why is the past perfect used in this sentence? Please tell me, thanks.

I went through it. I did find it a little hard to agree with past perfect. Even now I have my doubts about it.
However, I was trying to look at it this way. The speaker, in the present. is referring to the time before he came back to consciousness.
