Past perfect

Hello,

I wonder whether 'could have' is followed by past perfect? For instance: The medicine could have killed them as quickly as it (had) cured him. Is (had) necessary here because of 'could have'?

2) Is 'that' necessary in these cases: I believe that man is evil and (that) nothing can change him. Normally, I see 'that' written twice in sentences like this where the writer describes two things and connects them. But I wonder whether the second that is necessary at all.

3) Should confuses me when i try to use it in past tense: They kept asking him to enlist because, in their eyes, every man should fight for his country.
Since the sentence is past tense, does 'should' have a place here? If not, how else can one structure it in past tense?

4) Reg. apostrophe, is it correct to say: everyone else's problem.
