past and present tense in one question?

good day allhow can  i arrange between past and present in one sentences ?are these sentences right ?do you know who first entered the class and opened the window ?did you know who first entered the class and opened the window ?do you know who first  had entered the class and opened the window?************the cat  had entered the house and went under the stairs.....i used (had+past participle)  to describe the first action but the action following it i used simple past.... is that right ?thanks

Did you know who first entered the class and opened the window ? Correct

Do you know who first had entered the class and opened the window? I think had is not necessary.

The cat had entered (also entered) the house and went under the stairs..... Here even enter can be in past. When the interval between the first action and the second is quite short, past tense can be used for both.

i used (had+past participle) to describe the first action but the action following it i used simple past.... is that right ?
