Passive VS past simple

Imagine you see a picture frame dropped on the floor and you want to figure out what happened. I was wondering which of the followings sounds correct.

A. How did it drop?

B. How was it dropped?

Ryann which of the followings following

(A) implies that it fell down by itself. Assuming that it fell off a wall, "How did it fall down?" seems more likely though.

(B) implies that a person dropped it.
