As for your original post, yes, that's true.
The passive voice is indicated by a form of the verb "to be", so the question form would have to begin with a form of "to be" (or a form of "to have" for the perfect forms of "to be", or a modal, of course). In any case, forms of "to do" would not occur in that position.
