“My hand hurts” is fine too. It probably is more common than "my hand is painful."

I agree - it's a far more common way to say you "hurt." (At least, AmE.)

My hand hurts - that cut I got is very painful.

My head hurts - this headache I have is really painful.

However, you CAN say "my hand is really painful right now."

What about "my foot is really painful." or "my head is really painful?"
