out / out of - choices

Could someone please decide whether my choices of prepositions below are OK or not?

1) The witch flew out the window.
2) The cat walked out the door.
3) The cat walked out ouf the house.
4) All her hopes have gone out the window.

5) The witch threw the cat out the window.
6) The witch threw the cat out the door.
7) The witch threw the cat out of the house.

Sometimes, I see expressions like "throw/hurl out of the window". Are these correct? From a purely logical point of view, doesn't "out of the window" mean "from inside the wooden/plastic structure (or the glass panes) of the window? :-)
