We need more context. If a machine, it means that it is broken or operating incorrectly. If it a dimension, it means that it is incorrect. If it is a person, it may mean than she is upset or emotionally unstable.

I thought at first an "incorrect dimension" might be like a warp in the space-time continuum -- then I realized you just meant that something had been measured wrong.
