I'd say "of" rather than "off" ..

- We better get out of the elevator now.

Or ..

- We'd better get out the lift now.

I assume you're a native British English speaker. Are you saying its okay to say get out of the lift or elevator?

It is the 'we better' that is not generally acceptable. I presume that 'off' was a typo?

In writing, I might put:

- We'd better get out of the lift

In speech:

- We better get out the lift

(It's not substandard, it's elision - the sound of the 'd' is so close to the 'b' that people omit it)

That's a good question. There doesn't seem to be any logic to it. I would say:

- I'm getting off the train at Southwick
- We all had to get off the plane at Stuttgart

But ...

- Get out the car. Fast!
- I got out the lift at the fifteenth floor
