Generally speaking, I understand what the word "otherwise" mean but I don't quite understand the sentences below, can anyone explain to me? Thanks in advance!!!

a. "Non-pharmacological management of fever in otherwise healthy children."

b. "The successful rate of this operation is quite high in patient with otherwise healthy heart."
