Hi,
This is from a radio traiscript which has been bothering me for some time. Would you take a look, please?

[Dr. Linda Peeno, Medical Director at Humana] I denied a man a necessary operation that would have saved his life and thus caused his death. No person and no group has held me accountable for this because, what I did was, I saved a company a half a million dollars.
[Anchor] Linda Peeno goes on to say, the mantra there [at insulance company Humana] was, we're not denying care, we're denying payment.
[Correspondent] Which is, in effect, denying care when you're otherwise hitting people with bills they can't pay.

My question is about the last sentence. I think I understand what it means, but still, to me, it's quite difficult to grasp this usage of "otherwise." How would you paraphrase this last sentence...? I'm at a loss, I'll appreciate any input. Thanks!
