On Tuesday, May 6th, 2012, I .....

Does anybody frown on using the ordinal suffix ('st', 'nd', 'rd' or 'th') with numbers in dates in a formal report--yes or no?

I do, but you should consult the style guide of your choice (or the one used by your publisher) for anything that's going into print.
