order of the actions?

"He was exhausted because he hadn't been sleeping well since he hurt his back."

order of the events:

1) He hurt his back

2) He hadn't slept well

3) He was exhausted.

event (1): Comes before event (3) and event (2)

so shouldn't it be edited :

*He was exhausted because he hadnt been sleeping well since he HAD hurt his back.

I prefer your version (with a ' in hadn't, of course) However, cue words like "since" or "after" let you know the sequence, so you don't have to rely on tense to tell you.

For the record, I took the example from a grammar book in which Past Perfect Tense sequence rules are strictly applied next to the cue words like "since", "after" .That caused confusion.
