Or else vs. otherwise

"You will do it or else you will die!"

"You will do it, otherwise you will die!"

It has occured to me once whether those words are synonyms in meaning in the sentences above?

I am sorry for the clumsy and stilted sentences I've though of.

Used this way, they are synonyms. To me, though, the first sentence seems more threatening, but that's only because I don't know the context.

E.g.,

You will do it, or else you will die! I will kill you if you don't do what I want you to do.
You will do it, otherwise you will die! You will die if you don't take your medication.
