Are both correct? Is there any difference between the two?

"And" may be acceptable. I would not use it, except to say, "He's worse than Milosevic and Saddam combined!" - A.

So or compares the person with Milosevic and Saddam separately while and compares him to the total of Milosevic and Saddam, right?

That's my feeling, but other opinions should be obtained.

As I said, if I were using "and," I'd specify "combined." - like wearing suspenders and a belt.
