Opposite of superficial inuries

I know if someone is injured in an accident and the injury is only outward, we say "He got only superficial injuries".

Similarly, I wonder how to say if the injury is not seen outwardly, but still he was very much hurt internally.

People can also sustain internal injuries.

But these indicates as if it might be either physical or internal injuries or both. But does not specify it's an internal injury opposing to "superficial injury".

e.g. He sustained life-threatening internal injuries.

Opposite of superficial in terms of medical terminology is deep.
e.g. superficial muscles as opposed to deep muscles. This injury was superficial as opposed to deep. The deep tissue was affected by the viral infection as opposed to the superficial.
