(1) fatso - mostly used by kids+teenagers, not adults?
(2) it is NOT offensive (when addressed to a close friend... and in a jocular manner of course)?
(3) what other slang terms (synonymous with /close enough to 'fatso') are used in modern English?

In the US, you might hear the following hierarchy of terms for stoutness:

Extremely and aggressively offensive: pig, fat pig.

Offensive: fatso, fatty, porky, porker, beached whale, hippo, hog, butterball, butterbean, doughboy, slim (ironic), skinny (ironic), two-ton.

If you know the person well, the following might be almost an affectionate nickname or a term of endear
