Oops, what is that the subjunctive?

Hi, everyone, please read the following sentence:

SEC regulations require that a public company disclose to their potential investors any legal, technological, or financial complications that could endanger their investments.

My questions is: that a public company disclose(s)
Why hasn't put a (s) to the verb "disclose" as the singular subject (a public company) is at the front?

In other word, I can tell the present subjunctive is often used after verbs that express the idea of command like:
order, request, require, tell.

Is that any other verbs that give the commendable sense?

and is there any other form of subjunctive ?(past subjunctive, present perfect subjunctive?)

The following list contains the verbs which are typically used in the subjunctive mood. The rules dictates that the verb after "that" remains in its base form. Therefore the 3rd person singular rule is not applicable.
I advise (that) he start exercising and taking vitamins.
