I would agree with DSE; though soem writers might use "Then only did I realize..." for the sake of emphasis or variation. But it would not be common.

I'd like to add that in casual English, you will also hear people use the the second sentence. However, it would be a very different sort of usage from the first sentence. In the second sentence the word "only" would have a meaning similar to "but".
