ONE SENTENCE PLEASE HELP, can't figure it out

Which would one say

1 How many times have I needed a pan and it was dirty because you used it last and didn't wash it.

2 How many times have I needed a pan that is dirty because someone didn't watch after they use it

3 How many times had I needed a pan that was dirty because someone didn't wash it after they used it.

There is something wrong with 2, if one was trying to speak generally, how would one say it?

and are the others correct?

one more thing: could one say
