Some people say this to mean that they have an appointment at 1:15 pm.

An appointment that was going to start at 1:15 p.m. (presumably p.m. -- perhaps these people work the night shift).

It's not uncommon to use the time as a noun to refer to what is happening at that time.

For example, a receptionist may let a doctor know that her "2 o'clock is here" meaning the person who had the appointment for 2 p.m. has arrived.
