On mind vs. In mind

#1 Okay son, what's troubling you? Tell me what's on your mind.
#2 At this point, I don't know what the designers have in mind for the new project.

I think the above usage is correct for 'on mind' vs 'in mind' to mean something that you are thinking inside your brain. But is there any rules as to when you should pick one over the other? Anybody?

Something that is in mind is something you are thinking about. Something that is 'on your mind' is something you are worrying about, not just thinking about.

on my mind also has the meaning of: focus of thought/attention as in the song " Alway on My Mind " for example.
