The fact that he passed the exam is unbelievable.

I personally don't omit it, in speech or writing, and I don't like to hear, or see, it omitted, either.

Perhaps usage is different in the US and the UK. The fact he passed the exam is unbelievable sounds and looks fine to me, although "the fact that" is a sort of set phrase and in that particular sentence I think the "that" would possibly be more likely used than not.

"That" and other relatives are frequently omitted:

The man I saw yesterday was in the park again today.
