OK vs okay

Are OK and okay always completely interchangeable or would you use one over the other in some of the examples below? Is ok without capitalizing never acceptable?

- She was shocked but OK.

- "Everything's OK?"

- "I'll see you tomorrow, OK?"

- "Really? OK. Then just give it to him when he gets back."
