I think when it is followed by a noun there should be a comma, as you pause there when speaking.
I would say no. There really is little if any pause after saying "often" at the beginning of a sentence, and it's only one word for crying out loud. And there is no rule that if a pause occurs in speaking that a comma must appear in writing. If it adds clarity, then fine.
