The person who uses this expression obviously dislikes the woman. There could be many reasons for this dislike.

A lot of people find it offensive to call a woman a b-i-t-c-h.

The office b-i-t-c-h is the person in the office that no one likes because she is always correcting everyone or complaining about everyone's work.

It is not a nice term, and it is not nice to call another person that or to be called that

Yep, as I said, in certain cultures in the inner cities. And in prisons, but it has a slightly different meaning there.
