Could you please help me correct these sentences please?

1 You need to come up with a plan of/on/for how to do this.
2 This card will get you through/past all the gates.

3 I was astonished (at) how well he played.

4 One of the only times he (does) decided to go out, he loses his keys.
5 This casino offers as wide a variety/range of games as you're going to find worldwide. (how would you say this sentence?)

For 2, both are OK??

and for 4 Do you leave in DOES and if so wouldn't it be "does decide"

and for 5 do you leave in A "as wide A variety of games as"
