How should I read room numbers and street numbers? For example, 612(six twelve or six one two?)

There are no rules, only conventions, Summer. Read it as 'six twelve' the first time, which is the faster and perhaps more spontaneous way, then repeat it as 'six one two' if your listener asks for clarification or repetition.
