Where should I put 'and'?

I use only the first 'and', but there seems to be regional variation.

Most speakers of BrE wouldu not use the second 'and. It is possible to omit the word 'dollars' (or 'pounds) if it is clear from the context what is being talked about.

In BrE, and comes after the hundred. The example above would be spoken as: three hundred and forty-five thousand five hundred.

In the US you sometimes hear even more shortened versions of this, for example:

Three hundred forty-five thousand five.

Three forty-five thousand five (hundred).

Three forty-five five.

Three forty-five point five k. (The k means thousand.)
