Note about homework

If a student hasn't done her homework how would you write a note about it on student's diary?

1) Homework is not done. (This makes no sense.)

2) Homework has not done. (This looks akward)

3) Homework has not been done. (This is may be right; but I'm not sure)

Please tell me which one of these is correct. If none of them is correct, tell me what's the natives choice?
