Does this sentence require a comma?

These decisions affect not only individual victims and offenders, but families and communities as well.

Or

These decisions affect not only individual victims and offenders but families and communities as well.

Or does the "not only" require a "but also"

These decisions affect not only individual victims and offenders but also families and communities.
