Not only ..... but also

Q: A nurse must be paient. She must be kind,too.
I'd like to know whether I can rewrite the above sentence by using "not only ... but also" as follow.

1. A nurse must not only be patient but also be kind.
2. A nurse must not only be patient but also kind.
3. A nurse must be not only patient but also kind.
4. A nurse not only must be patient but also must be kind.
5. A nurse not only must be patient but also kind.
