Who says what? Right. " I'm wondering if actually it should be like this: A: Have you failed?

Who says what? The way you've written it, it seems as if one person says "Have you failed?" and another says "Never ....Right. we can stop here, today." I'm wondering if actually it should be like this:
A: Have you failed?
B: Never.
A: Right, we can stop here today.
Or perhaps one person says all of it, and the word "Never" is echoing someone's reply or imagined reply?
