Neither-either?

Hi, I'm not sure whether this sentence is correct or not;
I'don't want my mobile phone number neither my address to be mentioned. Can you please rewrite it using either or neither if it's wrong? Thank you.
PS, is correct to say; I don't want my number or my address mentioned? Thanks.

And I'm not sure if you noticed that I'd dropped "to be" before mentioned in the sentence that you said it was OK, is it necessary to be "to be mentioned" or can I just drop it?
