It would be "needs". But I don't really understand your sentence beginning with "Please confirm...". You mean paying for the t-shirt?

I don't understand the second part of the sentence either.
Please confirm that you have received the t-shirt? Or please confirm that I have to pay for it? Or... what?

i would phrase my sentence this way, "Please advise if it needs to be paid?"

You could say: "Please let me now if it/the t-shirt needs to be paid/has to be paid /must be paid.
