The skull (and cross bones) is a symbol of death. It is "death's head".

The author, like everyone else, has a skull in (behind, within) his face. The face, being flesh and not bone, decays after death, leaving only the skull.

The author asks if he really needs to be reminded by the tolling (ring) of funeral bells that he will die some day when his own skull is reminder enough.
